Postpartum Care Intake Form

Thank you for your interest in receiving postpartum care with Carrie Sizer, RM at Vernon Midwifery. This form is for families who have recently given birth or are planning to deliver soon and would like support during the early postpartum period. You do not need to have received prenatal or birth care from Vernon Midwifery to access this service.

Postpartum care typically continues for about six weeks after birth and includes:

  • Visits in your home, at the clinic, or in hospital
  • Newborn health monitoring
  • Support with breastfeeding, pumping, or formula feeding
  • Emotional wellness check-ins and mental health screening
  • Physical recovery monitoring for birthing parents
  • On-call access for urgent concerns

Postpartum care is available for clients who gave birth at Vernon Jubilee Hospital, other hospitals, or at home with a different provider.

Before you fill out the form

Please read the following important details:

  • MSP coverage: Postpartum care is fully covered for BC residents with valid MSP coverage, even if you gave birth with a doctor or OB. If you do not have MSP, private care is available. If you do not have MSP, please enter a zero in the Personal Health Number field in the form below.
  • Location: Postpartum visits are held at your home or at The Health Nest clinic in Vernon as needed.
  • Limited availability: Spots for postpartum-only care are limited. Submitting a form does not guarantee care, but we will do our best to accommodate your request.
  • No referral required: You do not need a doctor’s referral to access postpartum care.

After you submit your form, we’ll review your information and contact you by phone or email within 5 business days if space is available for your due month. If we’re currently full, you may be added to a waitlist, and we’ll follow up if a spot opens up.
