Thank you for your interest in receiving full midwifery care with Carrie Sizer, RM at Vernon Midwifery. This form is for people who are newly pregnant and looking for full-scope midwifery care, from early pregnancy through birth and postpartum.

Midwifery care includes:

  • Prenatal visits throughout your pregnancy at The Health Nest clinic in Vernon
  • Birth support at Vernon Jubilee Hospital
  • Postpartum care for you and your baby for about six weeks after birth
  • Feeding and lactation support
  • Emotional wellness check-ins
  • On-call access for urgent concerns

Before you fill out the form

Please read the following important information:

  • MSP coverage: If you are a BC resident with valid MSP coverage, care is fully funded. If you do not have MSP, private care is available. If you do not have MSP, please enter a zero in the Personal Health Number field in the form below.
  • Location: Prenatal and postpartum visits are held at The Health Nest clinic in Vernon or in your home as needed.
  • Hospital births only: Home birth services are not available at this time.
  • Limited availability: Due to high demand, care spots are limited. We encourage early submissions, but cannot guarantee space.
  • No referral required: You do not need a doctor’s referral to access midwifery care.

What happens next

After you submit your form, we’ll review your information and contact you by phone or email within 5 business days if space is available for your due month. If we’re currently full, you may be added to a waitlist, and we’ll follow up if a spot opens up.

You are welcome to submit the form at any point in your pregnancy. Earlier is better, but late referrals are considered as space allows.

Midwifery care is in high demand and spaces are limited. We recommend submitting your intake form as early as possible – ideally as soon as you have a positive home pregnancy test. If we’re unable to offer full midwifery care based on your estimated due date, you may still be eligible for postpartum-only care.
